Preamble

 

Interested competent and reputable Developers are invited to apply for the Pre- Qualification Exercise as a condition to be condition to the participation in the development of the University Land measuring 4, 702,25m2 located in Mabushi District of Abuja. The portion of land lying between coordinates 1005072.630MN and 330003.88ME and 1005072.346MN and 330078.949ME is delineated by beacons PB 814, PB 815, PB 846 and PB 84. The survey plan is available with the Director of Works and Physical Planning.

 

Requirements:

1.       Compulsory and Mandatory Criteria:

i.        Evidence of registration with the Corporate1 Affairs Commission (CAC).

ii.       Company Audited Account for the least three years duly stamped.

iii.      Evidence of Tax Clearance Certificate for the last three (3) years.

iv.      Evidence of compliance with the provisions of the Pension Reform Act, 2004 and Industrial Training Fund (ITF) Amendment Act, 2011 as applicable.

v.       Evidence of Value Added Tax (VAT) registration

vi.      A sworn affidavit indicating that all documents submitted are genuine and verifiable.

vii.     A sworn affidavit that none of the Directors has been convicted in any country for any criminal offence, including fraud or financial impropriety.

viii.    A sworn affidavit indicating that no current staff of the University of Lagos is a former or present Director, shareholder or has any pecuniary interest in the company submitting bid.

 

Failure to provide any of the above listed documents will automatically disqualify the Developer.

 

2.    Other Requirements:

i.        List and Sources of Equipment to be used for the project.

ii.       Indication of Technical and Administrative staff strength, with academic/professional qualifications and years of experience of key personnel available for the project.

iii.      Bank reference from a reputable Bank.

iv.      Verifiable evidence of similar jobs successfully executed or being executed in Nigeria over the last five years stating, type, cost and evidence of award and completion.

v.       Evidence of Social Responsibility in terms of Endowment, Sports or ETF contribution.

vi.      Evidence of payment of non-refundable pre-qualification documentation fee of Thirty Thousand Naira (N30,000.00) in bank draft payable to the University of Lagos.

All the documents should be sealed in an envelope should be marked “Confidential and written: Notice of intention to develop University of Lagos Mabushi Abuja Land’ and delivered either by reach the address stated below not later than 12 Noon on 22nd April 2013:

3.       That participating in Pre-qualification Exercise shall not be considered as a commitment on the part of the University, nor shall it entitle any Developer to any claim whatsoever to seek indemnity from the University Lagos.

The Lagos University Teaching Hospital desires to procure drugs for the Pharmacy Department of the Hospital with a view to improving patient care services in the Hospital.

 

In consideration of the above:

 

(1)     Invitation for submission of Pre-Qualification documents are being invited from interested reputable companies who wish to tender for the above supply.

 

(2)     Invitation to tender financial bids for the above supply from reputable companies who pre-qualified in the pre-qualification exercise, will be conducted at a later date.

 

Submission of Pre-Qualifications Documents

2.       Company’s profile indicating the Management profile including the names and professions of the Top Management Staff.

3.       3 Years (Current) income Tax Clearance Certificate.

6.       Bankers reference.

7.       Current annual License to practice as Pharmaceutical Chemist of the Company’s Superintendent Pharmacist.

8.       Certificate of Incorporation.

9.       Registration with Pension Administrator.

10.     Evidence of contribution to Industrial Training Fund.

11.     Any other relevant information.

 

Completed Pre-Qualification Document marked “Pre-qualification Documents for Technical Bid for Supply of Drugs at LUTH” must be submitted in sealed envelope and addressed to:

8.       Evidence of compliance with section 8(1) (3) of Industrial Training Fund Amendment Act 2011.

10.     Sworn affidavit affirming that information being provided by bidders is correct

A. Introduction

Family Health International (FHI 360) with funding from the United States Agency for International Development (USAID) is supporting the government of Nigeria (GON) to finance sectional cross sectional integration of HIV/AIDS and TB services by building Nigerian capacity to deliver sustainable high quality, comprehensive, prevention treatment, care and related services in various states within Nigeria. FHI 360 hereby invites reputable companies with proven experience and expertise in the supply of laboratory items (ancillaries, equipment and consumables) to participate in the prequalification process as described below.

 

B.      Scope of Supply

The Scope of supply involves delivery of various laboratory items (ancillaries, equipment and consumables). Interested companies should write to procurement@sidhas.org for the list of items.

 

C:      Qualification Criteria

The minimum eligibility criteria to participate in the prequalification process shall be as under and should be submitted along the price quotation in a sealed envelope

1.       Evidence of registration with Corporate Affairs Commission and other bodies with copies of form CO2 and CO7

2.      Evidence of payment of tax for the last three years

3.      Detailed company profile with registered address functional contact email, telephone numbers and point of contact for the organization

4.      Verifiable evidence of similar jobs successfully executed in the past for an amount equal to or above 14 million Naira (including copy of Purchase Order/contract and signed delivery note)

5.      Recommendation letter from the Bank stating ability to execute such supply.

6.      Current company’s audited statement of accounts

7       Affiliation with any international organization or original equipment manufacturer.

8.       Provide recommendation letter of supply from past organization for successful completion of supply order.
