3.       Pre-Qualification

The bids must contain the following:

i)                   Evidence of registration with Nasarawa  SUBEB as contractor(s)

ii)                Evidence   of      Incorporation/registration   of Business name with CAC

iii)              Current Tax Clearance

iv)              Veritable   evidence   of   similar   procurement experience, if any.

Bidders may bid for one or 2 lots; and discounts offered for the award of combined lots will be considered.

4.       Interested eligible bidders may obtain further information from and inspect the bidding documents at the address given below between the hours of 9.00am-4.00pm   (Monday-Thursday)   and   9.00am – 1.00 pm(Fridays):

CapacityPlus is a USAID-supported global health workforce project led by Intrahealth International Initiative. CapacityPlus is funded in Nigeria through a range of HSS, MCH/POP and OVC allocation by USAID mission to develop and support an integrated program of HRH strengthening, implemented at both national and sub-national levels. The overall goal of this program is to increase the availability of health workers to meet the HIV/AIDS, Maternal and Child Health (MCH), reproductive health (RH) and other priority health needs of scalable HRH interventions. Implementation is targeted at both Federal and State levels through active engagement with a range of agencies, including Federal and Ministries of Health, the National Primary Health care Development Agency (NPHCDA), training institutes, and regulatory councils.

CapacityPlus, in partnership with the NPHCDA, wishes to carry out a study on attrition, availability and Retention of Midwife Service Scheme Officers in Nigeria between 2009 and 2012. The purpose of this solicitation is to contract the services of a research firm/company with requisite technical experience in qualitative and quantitative research techniques to work with CapacityPlus and the NPHCDA to conduct the proposed study, the result of which will guide programming to reduce attrition, attract and retain midwives in areas of greatest need. The successful firm shall have the responsibility of putting together a team of qualified personnel to complete this assignment.

The offerers should have:

a)       Legal status approved by the government of Nigeria enabling it to carry out the tasks mentioned above;

b)      Internal expertise to develop the tools; collect key data; and perform interviews with key informants;

c)       A proven professional record in the area of health systems research, preferably in developing countries;

3.       Mandatory Requirements without which a Bidder shall be disqualified:

Prospective bidders are required to submit the following documents:

i.        Certificate of Incorporation/ Evidence of Registration with the Corporate Affairs Commission (CAC) and Articles of Memorandum of Association.

ii.       Evidence of Certification /Exemption from PENCOM in accordance with the Pension Reform Act. 2004 including evidence of remittances of Employees’ Pension Contributions/ Deductions.

iii.      Evidence of Tax Clearance Certificates for the last three years, (2010, 2011 & 2012) corresponding to declared turnover.

iv.      Evidence of VAT Registration with TIN No and past remittances for the last 3 years.

v.       Evidence of contribution/Remittance of 1% of Annual Total Emolument accrued to members of staff of the company payable to ITF.

Other Requirements:

vi.      Company Audited Accounts prepared by a reputable Chartered Accounting firm for the last three years (2010, 2011 & 2012) showing annual turnover;

vii.     Evidence of Technical Capability and list of registered professionals for Construction/ Renovation works.

viii.    Verifiable list of similar jobs successfully executed in the last three years including letters of award of contracts, project costs, job completion certificates and payments (please provide contact addresses of the Clients who may be contacted for reference).

ix.      Evidence of Financial Capability to execute the Projects (Bank Guarantees from reputable banks will form part of the evaluation criteria).

x.       Manufacturer’s Authorization from the approved Manufacturers listed in the Policy for Procurement/ List of Manufacturers and Equipment Handbook (for medical equipment).

4.       Collection of Tender Documents

Tender documents are obtainable from the Office of the Ag. Secretary of the Tenders Board Room 04 on the presentation of a copy and original receipt of non- refundable Tender fee of N20, 000.00 (Twenty Thousand Naira) Bank Draft issued in favour of National Obstetric Fistula Centre for each lot.

5.       Submission of Tender Documents

i.        Completed Technical and Financial proposals (three sets of hard copy and i soft copy) shall be submitted in two different scaled tamper proof envelopes and labeled “Technical Proposal” and “Financial Proposal” and both placed in a third envelope. All the 3nos envelopes should bear the “Project Name” and Lot No. at the Top Right Hand Corner of the Envelope and addressed to:
