A. INTRODUCTION

Family Health International (FHI 360) with funding from the United States Agency for International Development (USAID) is supporting the government of Nigeria (GON) to enhance sustained cross-sectional integration of HIV/AIDS and TB services by building Nigerian capacity to deliver sustainable high quality, comprehensive, prevention , treatment, care and related services in various states within Nigeria.

 

The Nigeria Country Office hereby invites reputable suppliers of office equipment to submit quotations for the supply of office equipment, as specified in Section B (Scope of Work) below.

 

B.SCOPE OF WORK/DELIVERABLES

C. Selection Criteria

The minimum eligibility criteria to participate in the prequalification process shall be as under and should be submitted along the price quotation in a sealed envelope Evidence of registration with Corporate Affairs Commission and other bodies with copies of form COT

 

  1. Evidence of payment of tax for the last three years
  2. Detailed Company Profile with registered address, functional email, telephone numbers and point of contact for the organization.
  3. Verifiable evidence of similar jobs successfully executed in the past for an amount equal to or above 14 million Naira (including copy of job order and signed delivery note)
  4. Resumes/CVs of key technical staff, with copies of professional certificates endorsed or initialed by the staff
  5. Recommendation Letter from the Bank stating ability to execute such supply.
  6. Current company’s audited statement of accounts
  7. Evidence of affiliation with any international organization or original equipment manufacturer
  8. Provide recommendation letter of supply from past Organization for successful completion of supply orders.

 

D. Instruction to Prospective Bidders:

The Vendor must have experience in the field of supplying item (Part B : Scope of Supply) and below are instruction to be strictly complied with.

 

All quotations must be in Naira (NGN) and signed by Authorized representative of the bidding company.

 

Quotations must provide the following information:

  1. Delivery Schedule
  2. Payment terms
  3. Validity of Quote _
  4. Warranty of Equipment
  5. Cost of equipment maintenance on expiration of warranty period.
  6. Complete list of copier consumables/spare parts, and associated costs

 

E.      Deadline for Submission

All applicants submitting a Proposal must submit their proposal to:

All documents required by this RFQ must be received at the above designated location NO LATER THAN 2:00PM on Tuesday July 09, 2013

 

No Facsimile (fax), e-mail documents or postmarked documents will be accepted.

Issuance of this RFQ in no way constitutes a commitment by FHI360 to award a contract.

 

FHI 360 Claims the right to vary number of items at time of award or split the award as may be deemed necessary in terms of Quantity to be delivered.

FHI 360 is Value added tax (VAT) exempted, but charges Withholding Tax (WTH) in compliance with Relevant Nigerian laws.

INTRODUCTION

In compliance with the policy of the Federal Government of Nigeria on Health Insurance Scheme, the Federal University of Agriculture. Abeokuta (FUNAAB). intends to embark on the implementation of the Tertiary Institution Students’ Health Insurance Programme (TISHIP), for the 2013/2014 academic session.

 

The main objectives of this Consultancy are:

  1. Provision of adequate healthcare for all students
  2. Ensuring  Students  well being  and  development  health wise   by   utilizing   the   facilities   of   the University Health Centre and appointed Clinics where applicable;
  3. Managing Health Plans for the Institution.
  4. Managing Medical Service Database for the Institution:
  5. Providing Adequate Supervision and Coordination of the Students’ health activities

 

The Federal University of Agriculture, Abeokuta now invites eligible Health Management Organisations to express interest in providing the above services. Interested Health Management Organisations must provide information indicating that they are qualified to provide the services listed above.

 

The information to be provided by the interested Health Management Organisations are:

 

  • Company profile/brochure:
  • Certificate of Registration with C.A.C;
  • Description of Similar Assignments with Names and Contact Address of the Clients. E – mail Addresses, Telephone Numbers and the date of the execution of the services;
  • Availability of appropriate skills among Staff;
  • Availability of essential technology;

Prospective bidders are required to submit the following documents:-

i.            Certificate of Incorporation/ Evidence of Registration with the Corporate Affairs Commission (CAC).

ii.            Evidence of Certification /Exemption from PENCOM in accordance with the Pension Reform Act, 2004 including evidence of remittances of Employees’ Pension Contributions/ Deductions for those firms and companies with five or more employees.

iii.            Evidence of Tax Clearance Certificates for the last three years, (2011, 2012 & 2013) corresponding to declared turnover.

iv.            Evidence of VAT Registration with TIN No and past remittances for the last 3 years.

v.            Evidence of contribution/Remittance of 1% of Annual Total Emolument accrued to members of staff of the company payable to ITF.

vi.         Company Audited Accounts prepared by a reputable Accounting firm for the last three years (2011, 2012 & 2013).

vii.        Evidence of Technical Capability and list of registered professionals for Construction/ Renovation works.

viii.       Verifiable list of similar jobs successfully executed in the last three years including letters of award of contracts, project costs, job completion certificates and payments.

ix.          Evidence   of   Financial   Capability   to   execute   the Projects.

x.           Company Profile and Technical qualifications/Curriculum vitae of key personnel with evidence of experience on similar jobs.

xi.            For lot 1 (A-C). Companies must be Franchised and are within the guide line as prescribed by the Bureau tor Public Procurement.

2.       Submission of Requirements:

i.        Interested contractors should submit their documents with evidence of payment of non refundable fee of Ten Thousand Naira (N 10,000.00) for supply and Twenty Thousand Naira (N20,000.00) for construction works in favour of Federal Medical Centre, Nguru.

ii.       The Documents should be enclosed in a sealed, tamper-proof envelope indicating the Lot at the Top Right Hand Corner of the Envelope and addressed to:

iii.      The documents should be deposited in the Tender Box at the office of the Procurement Officer, Federal Medical Centre, Nguru at its permanent site. Late submissions shall not be entertained.

 

3.       Opening:

Opening of the received documents will commence at 12.00 noon same day in the Board Room of the Federal Medical Centre Nguru. Representatives of firms/companies, Relevant Professional Bodies and NGOs are invited to witness the public opening exercise accordingly.

 

Please Note That:

i.            Only pre-qualified companies/firms at this stage will be invited for the next stage of the bidding process where applicable.

ii.            The Federal Medical Centre Nguru reserves the right to reject any or all the submissions that do not meet up with the requirements as specified.

iii.            The Evaluation of the received documents shall be strictly based on the requirements specified above.

Requirements;
