1.0     Background

 

The Lagos State University Teaching Hospital, Ikeja is the only State-owned Tertiary Institution providing Specialist Healthcare to Lagosians. Established in 1999 with a vision is to provide high quality health care services in a friendly environment where patients’ satisfaction is the ultimate. The 660-bedded facility offers services in multitude of specialties.

 

Sprawled across several acres of land, the Institution consists of various buildings housing the various out­patients, surgical, diagnostic and administrative services.

 

In compliance with international best practices, LASUTH wishes to continue the culture of facility management to ensure professional maintenance of its structures.

 

2.0.    Objectives

a.       To provide the required facility management support to facilitate the efficient delivery of the facility’s core function of providing tertiary health care.

b.       To attain clean, health hygienic and safe environment, including regular landscaping of gardens and lawns

c.       To attain stable power supply and proper maintenance and repairs of electrical power supply equipment and facilities in the Centres

d.       To attain the maintenance and repairs of all loose and fixed office/building equipment

 

 

3.0     Scope of Services

Though not all-inclusive, the following description of requirements should be used as a guideline describing the minimum of the maintenance and repair services which will be required of the contractor:

4.0.    Requirements

 

All intending Facility managers are expected to have relevant Professional        experience in the managerial/maintenance of a Health Facility with similar capacity. In addition, each Applicant will provide:

 

  1. Certificate of Incorporation/Registration with Corporate Affairs Commission including Article of Association
  2. Current Tax clearance Certificate for the last three (3) years including those of the Directors
  3. Evidence of Company’s financial Standing and 3 audited financial report for the last 3 years.
  4. Evidence of current Registration with State Consultancy Board/State Tenders Board in appropriate category of consultant/Contractor.
  5. Profile of organizations including list and relevant Academic Qualification(s) of Directors
  6. Evidence of successful facilities management with maintenance skill in a Health Facility with similar capacity of medicine of international /local standard with a minimum of (5) five years experience in such institution.
  7. Evidence of previous but similar job done in the last 3 (three) years.
  8. Evidence of registration with relevant Professional Body (ies) of both the organization and individual Directors.
  9. Evidence of registration with relevant Government Ministries or Agencies.

 

 

5.0.    General Information

6.0.    Submission

Application for Airlift of Pilgrims under State Pilgrims Welfare Boards/Agencies/Commissions for 2013 Hajj

 

Interested domestic/international airlines/carriers with requisite operating authorization and experience (who have not applied for the airlift of International/Tour Operators’ pilgrims) are hereby invited to submit applications for airlift of pilgrims under State Muslim Pilgrims’ Welfare Boards/Agencies/Commissions from Nigeria to and from Saudi Arabia for 2013 Hajj.

 

 

2.       The outbound airlift of about 85,000 intending Nigerian pilgrims to Saudi Arabia is expected to commence during the first week of September, 2013, while the inbound (return journey) is expected to commence during the third week of October, 2013. The airlift operations will be carried out at twelve (12) designated Hajj airports.

 

 

3.       Applicants are to note that Boeing 747-200 and newer series aircraft with capacity to carry a minimum of 450 passengers or other wide-bodied aircraft with capacity to carry a minimum of 300 passengers are required for the operation, All the aircraft must meet the safety requirements of the Nigerian Civil Aviation Authority (NCAA) and the Saudi Arabian General Authority of Civil Aviation (GACA).

 

 

Conditions of Application:

4.       Each applicant must possess a valid Air Transport Licence (ATL)/Air Operator Certificate (AOC).

 

5.       Applicants are also required to submit the following:

 

  1. Evidence of Incorporation/Registration with Corporate Affairs Commission (CAC).
  2. Certified true copy of CAC forms C02 and C07.
  3. Copies of relevant licences
  4. Evidence of financial capability to fund the airlift operation.
  5. Three years audited company Financial Account
  6. Three years Tax Clearance Certificates.
  7. Evidence of non-indebtedness to relevant aviation authorities and service providers either in Nigeria or in Saudi Arabia. Specifically, evidence of clearance from indebtedness to NCAA, NAMA, FAAN and SAHCOL or NAHCO must be provided
  8. Details/specification of aircraft to be used
  9. Verifiable written evidence of ownership or partnership/agreement with the owners of aircraft as confirmed by the NCAA.
  10. Evidence of ability to make available an authentic flight schedule for the operation not less than four weeks before the commencement of airlift.
  11. Proposal for luggage and Passenger Screening (Security) arrangements in Saudi Arabia.
  12. Proposal for in-flight Catering Services in Nigeria and Saudi Arabia with details name(s) and addresses of caterers clearly stated and subject to inspection and approval by relevant regulatory authorities and the Commission.
  13. Proposal for Crew accommodation during Hajj.
  14. Arrangements for over flight clearance, en-route and landing permit.
  15. Proposal for Ground Handling services in Nigeria and Saudi Arabia.
  16. Arrangements with fuel suppliers in Nigeria and Saudi Arabia.
  17. Evidence of installation of EGPWS as required by NCAA and GACA.
  18. Evidence of installation of an aircraft tracker/transponder as confirmed by NCAA.
  19. Organizational structure with names, qualifications and schedule of duties of the key staff of the company.
  20. Insurance which must meet the minimum third party (hull, passenger, baggage/cargo and third party) liability limit set by NCAA.
  21. Foreign applicants are also required to submit the above mentioned documents or its equivalents as required by ICAO.

 

 

6.  No applicant, irrespective of size and number of aircraft it intends to deploy, will be allowed to apply and/or airlift more than 40,000 pilgrims from a maximum of six (6) Hajj designated airports.

7.       Prospective applicants shall indicate their interest by:

ii.       Pay a non-refundable application fee of N1, 000,000.00 (one million Naira) in certified bank draft payable to the National Hajj Commission of Nigeria.

iii.      Collect an application form from the Secretary of the Commission at the Commission headquarters on the above mentioned address.

iv.      Complete and submit the Application form along with copies of all relevant documents on or before the deadline indicated on the application form.

v.       Appear for screening on a date to be communicated after the receipt of the application.

7.       Duly completed Application Form must be submitted to The Secretary, National Hajj Commission of Nigeria, Sokoto House, No. 1 First Avenue, Off Ahmadu Bello Way, Central Business District, and Abuja not later than 14th March, 2013.

 

8.       Successful Carriers shall pay Hajj licensing fee (Certificate) of two million Naira (N2, 000,000.00) to the Commission.

 

9.       Please be informed that this application is not intended to create any form of contractual relationship or obligations on the part of NAHCON. Consequently, NAHCON shall not be liable to any Applicant for any expenses or loss incurred in connection with this application. NAHCON will contact the Pre-qualified Applicants for interview, while formal Agreements will be signed with successful Applicants.

Background

UNICEF Nigeria Country Office plans to establish a Long Term Agreement (LTA) with an international organization to manage the provision of the professional services of vaccine security social mobilization. Learning and girls education, water and environmental sanitation consults and other professionals to support UNICEF programmes with the Government of Nigeria in its goals of  eradicating polio and strengthening routine immunizations, Improvement of learning and girls education and provision of safe water sources. Estimated number of consultants for 2013 is 540 and the LTA duration is for up to two (2) years.

 

Major Tasks

1.       Manage a pool of consultants and technical experts for planning, implementation, monitoring and evaluation of specific activities at the federal, state and LGA levels per the individual work schedules.

2.       Organize the movement of these consultants throughout the states and LGAs and FCT for effective coverage of programme activities as specified in programme schedules.

3.       Provide a monthly summary report of all activities and expenditures on authorized activities of personnel

 

 

Qualifications, Specialized Knowledge and Experience

  1. Reputed NGO or Consultancy/Management organization with experience of large scale project management and human resources management in social mobilization activities of development programmes
  2. Capacity to handle disbursements of large sums of money with experience in field monitoring and investigations.

 

Mandatory Requirements for Pre-Qualification

  1. 1.     Minimum of 5 years of existence: submit legal Registration Certificate,
  2. 2.     Minimum of 3 years of experience in provision of similar services and implementation of similar contracts: submit List and details of completed/ ongoing contracts in Nigeria, including certificates of satisfactory completion of similar projects.
  3. 3.     Financial Capacity of average annual turnover of USD1M for the last three years and Financial Sustainability: submit audited financial statement and/or credit rating report
  4. 4.     Personnel: office organogram and CVs of minimum of three (3) key staff

Gombe state Agency for the control of AIDS (GomSACA) coordinates the responses for HIV/AIDS epidemic in the state. It has received financing from the World Bank to coordinate the HIV/AIDS Program Development Project (HPDP 2) and intends to apply part of the proceeds for consultant services.

 

This is a call for Expression of Interest from qualified and competent organizations to respond to one specific service category – HIV Prevention of New Infections and care and support services.

 

Organizations are expected to work with any or all specified target populations within the specified geographic coverage areas to deliver, evidence based HIV intervention packages in line with international standard and best practices.

 

Also note that current target and geographic locations are based estimates, however findings from ongoing epidemic- appraisal will provide more accurate data, therefore CSO should be willing to accommodate such changes.

Selection Criteria

Gombe state Agency for the control of AIDS (GomSACA) invites eligible organizations to express their interest in providing any of the above mentioned services. Interested CSOs/PSOs must provide information indicating they are qualified to perform the services which include evidence of registration with CAC not less than one year old, brochures / organizational profile containing area of expertise,  description of similar assignments with  delivered results, demonstrate logistics, and infrastructural capacity to deliver, experience in similar projects, availability of adequate staff with appropriate skills, office accommodation   located within the state, operational governance structure and process, understanding the culture and traditional values including local languages of the project areas, evidence of working in and with the community and evidence of financial management. Organizations are encouraged to associate to enhance their qualifications.

 

 

General Terms and Conditions

Organizations will be selected in accordance with the procedures set out in the World Bank’s Guidelines: Selection and Employment of Consultants by World Bank Borrowers (January 2011 edition).

  • All Expression of Interest must be submitted in triplicates (three hard copies), one CD -Rom and delivered to Gombe State Agency for the Control of AIDS (GomSACA) not later than 27th February, 2013.
  • The sealed envelope must be clearly marked GomSACA EXPRESSION OF INTEREST FOR HAF GRANT.
