Ekiti State Government as part of her 8-point agenda is promoting prevention of preventable diseases, as well as delivery of prompt and qualitative Health Services to the people of Ekiti State.

 

In line with this, Ekiti State University Teaching Hospital proposes to carry out the under listed projects on PUBLIC-PRIVATE-PARTNERSHIP

 

(1)  LOT1  Establishment of Medical Gas Plant and Pipeline

(2)  LOT2 Construction of modern kitchen and cafeteria that befits a Teaching Hospital

 

Qualification Requirements

Interested partner who possess the following requirements are invited to submit technical and financial proposals.

  1. Evidence of registration with Corporate Affairs Commission (C.A.C)
  2. Evidence from the partner’s Bankers as to their financial capacity to undertake the project
  3. Evidence of tax clearance certificate of the last three years plus evidence of VAT payment
  4. Evidence of payment of Development levy in Ekiti State in the last three years.
  5. A comprehensive company profile to include a verifiable key professional staff and evidence of financial audited account for the last three years.
  6. Evidence of similar project undertaken
  7. Evidence of registration with professional bodies.
  8. Evidence of registration with the hospital

 

 

Applicants are expected to prepare an Architectural drawing of a modern kitchen and cafeteria that befits a Teaching Hospital.

The Companies submitting the pre-qualification and tender document should write the name and mobile phone number of a contract person at the bottom left corner of the envelopes.

This will allow for prompt communication.

3.0     Requirement for Tendering

All interested and qualified contractors should accompany their proposals with the following documents,

  4. Evidence of financial capability and Bank support,

4.0     Collection and Submission of Tender

Tender Bidding documents with detail specification can be collected from the Office of the Secretary, CCB upon presentation of evidence of payment of non-refundable tender fees of N50, 000.00.  Completed Tender Document should be submitted in sealed envelope marked “Tender for ……… (Indicate the specific project and Lot No. of interest)’. Addressed to the “Chairman, Code of Conduct Bureau, Abuja” and deposited into the tender box at the Office of the Secretary, CCB on or before 20th May, 2013.

 

 

5.0     Opening of Tender

The tender will be opened on Monday 20th May, 2013 at 11.00 a.m. at the CCB Conference Room. All bidders or their representatives, professional bodies, NGOs and interested stakeholders are invited to witness the bid opening.

2.0     Guidelines for Qualification:

The prospective contractors/suppliers are expected to submit their company’s Corporate Profile containing the following information/documents:

  1. Current Certificate of Incorporation by the Corporate Affairs Commission (CAC)
  2. Evidence of Registration with National Theatre
  3. Tax Clearance Certificate or the past 3 years
  4. VAT Registration Certificate
  5. Evidence of Relevance Experience
  6. Evidence of Financial Capabilities.
  7. Evidence that the company has fulfilled its obligations to Pension, Social Security contributions and Industrial Training Funds (ITF) as required by Pension Reform Act 2004 and ITF Amended Act 2001.
  8. Evidence of Tax identification Number (TIN)

 

3.0 Method of Application

The Tender Documents should be in sealed envelopes and marked with the title of projects of interest as appropriate separately for each lot (1 -9). Any envelope not appropriately labelled by specifying the lot will be rejected.

In addition to the requirements listed in our advertorial in the “Daily Trust” Newspaper of March 25th, 2013, interested Suppliers are required to comply with and submit additional documents in respect of the following statutory requirements:

 

1.       Evidence of Compliance with the Pensions Reform Act (as amended) regarding the registration of employees under the Act and remittance of contributions to the appropriate authority

 

2.       Evidence of Compliance with the Industrial Training Funds (ITF) Act No. 19 of 2011 with respect to payment and remittance of training contributions to the Fund for employees in their organizations; and
