Only successful Contractors in pre-qualification will be considered for the opening Tender.

The Oyo State Government as part of its healthcare reforms intends to pre-qualify consultants for the provision of comprehensive healthcare maintenance services in rural communities located within the state.

Accordingly, Oyo State Government hereby invites reputable, competent, and experienced Health Maintenance Organizations (HMOs) who have been licensed by the National Health Insurance Scheme to tender bids pre-qualification.

Scope of Work:

The scope of the Consultancy Works shall include but is not necessarily limited to provision of comprehensive healthcare maintenance to the following:

a)     The State Government employees and their dependants.

b)    Rural communities within the State.

c)     The aged and infant groups.

Pre-Qualification Requirements:

Interested firms should submit prequalification documents detailing the company’s capabilities and financial status as follows:

 

A.     General Company Profile

(i)      Name and address of company with telephone no., email, fax, etc.

(ii)     Legal Status (public liability company, corporation, etc.) registered capital, paid up capital, date of establishment, Nationality and country from which Control is exercised providing full details of ownership

(iii)    Evidence of Company’s operational license with the National Health. Insurance Scheme

(iv)   Evidence of Tax Payment-Tax Clearance Certificate for the three (3) years

 

B.     Technical Capacity and Capability

(v)     Detailed company profile including organizational structure, key professional staff with good knowledge base in Managed Health and qualifications.

(vi)    Verifiable evidence of having successfully carried out similar works in other States and Communities within Nigeria. Provide details of such works which are considered relevant in demonstrating ability and capability to successfully undertake this work to include:

a)     Title and Brief Description of Projects

b)    Client/State

c)     Scope of work performed

d)    Date of award of Contracts/Commencement

e)     Contractual period for execution of projects

f)      Actual period of execution

g)     Reasons for discrepancies between (e) and (f) above if any

h)    Nature of contracts (formal, informal, community etc.

C.     Financial Capability

(vii)   Company s Audited Accounts and Financial Reports for the past three (3) years

(ix)    Evidence of accessibility to Credit Facilities or additional funding from a reputable bank.

Submission of Pre-Qualification Requirements

The requested information and any supporting documents in respect of this requirement should be submitted in (1) original and 3 photocopies (hard copies) and electronic copy to the following email address: health@oyostate.gov.ng on or before 19th of June 2013.  The Oyo State Government of the Federal Republic of Nigeria herein­after called “The Employer” intends to carry out the Dualization of Agodi Gate Alakia -Airport Road in Ibadan.

2.0     The works involve the upgrading of the existing road to dual carriageway standard with raised median of 1.2m width, reinforced concrete lined drain and walkways on both sides and the provision of hydraulic structures including an underpass and street lighting. The pavement consists of lateritic sub-base, 150m thick crushed stone base and two layers of 50mm and 40mm thick asphaltic concrete binder course and wearing course respectively.

 

3.0     The Employer hereby invites sealed bids from contractors for the above works. Bidding is open to all reputable Civil Engineering construction companies registered with the Oyo State Ministry of Works and Transport in Category ‘F’ (Civil Engineering) or the Oyo State Bureau of Public Procurement, Category ‘E’.

 

Bid Requirements

Interested contractors are to submit the following documents

(i)      Evidence of registration with Oyo State Bureau of Public Procurement in relevant category.

(ii)     Evidence of the company registration with the Corporate Affairs Commission.

(iii)    Evidence of payment of personal Income Tax by at least two (2) Directors of the Company.

(iv)    Evidence of payment of pre-qualification or bidding fees

(v)     Evidence of successful completion of similar projects in the past (letters of award of contracts and certificates of completion)

(vi)    Company’s profile with relevant information.

(vii)   Registration of Business Premises from the Ministry of Trade, Investment and Cooperatives, Secretariat, Ibadan.

 

4.0     Bidding documents may be obtained by hand at the office of the Director of Highways, Oyo State Ministry of Works and Transport, Secretariat, Agodi, Ibadan on the production of treasury receipt for the payment of non-refundable fee of Two Hundred and Fifty Thousand Naira only (N250,000.00) between the hours of 08.00a.m. and 4.00p.m. on any working day starting from Wednesday 22nd May, 2013.

 

5.0     Bids shall be valid for a period of ninety (90) days after bid opening and must be accompanied by a Bid security of Ten Million Naira

(N10,000,000.00) and shall be delivered at the Office of the Secretary, State Tenders Board, Cabinet Department, Office of the Executive Governor, Oyo State Secretariat, Ibadan, Nigeria on or before 12.00noon on Tuesday 4th June, 2013 at which time they will be opened in the presence of bidders who wish to attend.
