Invitation for Expression of Interest for National Immunization Coverage Cluster Survey

1. Introduction

The National Primary Health Care Development Agency (NPHCDA) is a Federal Government Parastatal under the Federal Ministry of Health responsible for the development and strengthening of primary health care services through routine Immunization nationwide coverage. The NPHCDA is seeking the services of a consulting company or group of companies with a proven record of conducting large population-based health field surveys to undertake a national immunization coverage cluster survey.

1. Requirements:

Interested Company or group of Companies wishing to submit Expression of Interest are to forward their documents satisfying the following conditions:

1. Evidence of Registration with CAC. (As a limits company) (Photocopy)
2. Three (3) years Tax clearance from 2007 to 2009. (Photocopy)
3. Evidence of VAT registration with registration No. (Photocopy)
4. Company Audited account from 2007 to 2009. (Photocopies)
5. Comprehensive Company profile including registered office, functional contact address, GSM Number, and E-mail address etc.
6. Evidence of Technical experience/competence including evidence of jobs completed .(Verifiable evidence of having undertaken large scale population-based health survey)
7. Evidence of company/firm registration with relevant statutory regulatory bodies. (Photocopies)
8. The scientific human resources available to the company or group of companies in the areas of: field survey management, survey design, data collection, data analysis, mapping, report writing and financial management.

1. Submission Of Expressions Of Interest:

All interested Company or group of companies should submit their expression of interest in a sealed envelope marked “Expression of Interest for National   Immunization Coverage Cluster Survey” and addressed to:

Plot 681/682 Gimbiya Street, Off Port-Harcourt Crescent,

Area II- Garki- Abuja.

Closing date of submission is 16^th August, 2010.

All Expression of Interest Documents should be drooped in the Tender Box provided in the NPHCDA headquarters office complex.

1. The NPHCDA reserves the right to reject any document it considers to be of doubtful authenticity.
2. Late submission will not be entertained.
3. This notice shall not be construed to be a commitment on the part of the Agency to award any form of contract to any respondent nor shall it entitle any organization to claim any indemnity from the Agency.
