Invitations for submission of Pre-Qualification Tender Documents for Supply of Drugs
The Lagos University Teaching Hospital desires to procure drugs for the Pharmacy Department of the Hospital with a view to improving patient care services in the Hospital.
In consideration of the above:
(i) Invitation for submission of Pre-Qualification Documents are being invited from interested reputable companies who wish to tender for the above supply.
(ii) Invitation to tender financial bids for the above supply from reputable companies will be conducted at a later date.
Submission of Pre-Qualification Documents
Interested companies on the above are to submit “Pre-Qualification Document” which must include but not limited:
1. Formal Application
2. Company’s Profile indicating the management profile including the names and professional of the Top Management Staff.
3. 3 years (Current) Income Tax Clearance Certificate
4. 3 Years Company audited accounts
5. Certificate of Registration and Retention of Premises
6. Bankers reference
7. Current annual License to practice as Pharmaceutical Chemist of the Company’s Superintendent Pharmacist.
8. Certificate of Incorporation
9. Registration with Pension Administrator
10. Any other relevant information.
Completed Pre-Qualification Document marked “Pre-Qualification Documents for Technical Bid for supply of Drugs at Luth” must be submitted in sealed envelope and addressed to:

P.M.B. 12003, Lagos.
The Pre-Qualification Documents must be dropped in a Tender Box provided in the office of the Director of Administration on or before Friday 11th May 2012 by 12noon.
Opening of the Pre-Qualification documents will be on the same date at the new Board Room at 12.30 p.m. All interested companies are invited to attend the opening as scheduled above.
Tenderers should note that only Pre-Qualified companies will be invited to submit Financial Bids. The Lagos University Teaching Hospital reserves the right to reject any or all pre-qualification documents
